How to Find Your Out-of-Network Benefits
Step 1 — Call the Number on Your Insurance Card

Do I have out-of-network benefits for physical therapy?
What is my out-of-network deductible?
What percentage is reimbursed after my deductible is met? (your coinsurance)
Is reimbursement based on allowed amount or billed amount?
Step 3 — Request This Information in Writing

Step 4 — Submit Your Superbill
After your visit:
Receive your superbill from Rome Physical Therapy
Submit it to your insurance company
